We appreciate your interest in our school and are happy to help you
with our easy application process. For your convenience all the application forms
you will need are linked by grade level in the box on the right. The steps
below will guide you as to which forms you will need. Questions? Call the
academy office for help, 310-327-3094. We also recommend a visit to the CCA
campus before applying. STEP A: Complete the Applicant Information and Family Information forms. STEP B: The information
listed below, according to the applicant’s grade level in September, is
required along with the forms from STEP A. ●
Kindergarten: 1.
Child must be 5 years old on or before December 2nd. 2.
Completed Chancy
& Bruce Questionnaire form. ●
1st Grade: 1.
Kindergarten report card* (if Kindergarten was
attended). 2.
Completed Chancy
& Bruce Questionnaire form. ●
2nd-5th Grade: 1.
Most recent 2 years standardized test results.* 2.
Current report card plus last 2 years of report cards.* 3.
Teacher
Recommendation Form, Grades 2nd-5th (must be mailed directly from the
recommending teacher to CCA). 4.
Graded Writing sample. ●
6th-8th Grade: 1.
Most recent 2 years standardized test results.* 2.
Current report card plus last 2 years of report cards.* 3.
Middle School
Questionnaire form. 4.
Teacher
Recommendation Forms completed by the applicant’s current Math and Language Arts
teachers (each form must be mailed directly from the recommending teacher to
CCA). 5.
Graded Writing sample. * Some schools may require
an “Authorization For Release of Records” when requesting some of
the information above. We have included this form, with the other links in
the box above, for you to download and print if necessary. STEP
C: Starting the last week of February, you may submit all required forms and
documentation listed above for the next
school year to the Academy office along with the $75 non-refundable testing fee. Payment must be made with exact cash or money order payable to
CCA. (We also accept applications all year long for the current school year.) Next an appointment will be arranged for
the applicant to interview with the Head of School, followed by an
appointment for testing. STEP
D: After the interview and testing is
complete, notification letters will be sent home to inform you whether or not
the applicant has been accepted. Those applicants that are accepted will
receive a registration packet from the office. The registration packet must
be completed and submitted along with the $225 non-refundable registration fee in-order-to secure the
applicant’s enrollment for the next school year. Payment must be made
with exact cash or money order
